The forms you will need for your record book are the Personal Development Form (print one copy for each member) and the Annual Project Report Form (print one for each project each member is in). Link Buttons are below - just click and print!
For Example: If BillieJoMember is taking Sewing and Shooting Sports this year - then BillieJoMember will need two Annual Project Report Forms: One for Sewing and one for Shooting sports. AND BillieJoMember will need a Personal Development Form.
What's the difference? The Annual Project Report Form showcases what the member did in the project - detailing what was learned in each meeting. It also provides a record of expenses and incomes (if any) on the back side. The Personal Development Form showcases the member and what the member has done, earned or learned over the course of their 4-H year.
Many project leaders have the members fill in the Annual Project Report Forms at each meetings - so you might already have those started.
We will do a record book workshop in the Spring to help get books complete.
Record Books will be due by June 30th to the club.
